International 3-Week CCBT Program Cost

2013

This CCBT program, three weeks in length, will be used in conjunction with the audio CCBT therapy series "Overcoming Social Anxiety: Step by Step".

We will give you a bill (or HCFA form) to submit to your insurance company, if requested, and we may be able to file this for you.   It is highly recommended, and necessary, that you check with your insurance company FIRST, as most companies are doing their best today to avoid coverage of any kind for mental health care.  However, keep in mind your mental health care insurance SHOULD cover part of this treatment.  You may need to be persistent and assertive.

Dr. Richards is a licensed psychologist with a doctoral degree (Ph.D.) from an accredited institution (Arizona State University).  We will be glad to give you a "superbill" or HCFA form that you can send to your insurance company.  We will need you to pay us first.  Any remuneration received from your insurance company will be rebated to you, in accordance with law.

Payment methods: cash, check, money order, Visa, MasterCard, Discover.  Payment due on the first day of the group, OR 50% on the first day and 50% before the last session.

There is a $475.00 registration/application fee, which is refundable if you are not accepted into the program.

Regular Rate

The regular rate is $5,470 for the entire 3 week program, all-inclusive. A sliding-scale fee may also be used, if necessary (see below).

Sliding Scale Rate

The sliding fee scale is only for people who need to use it:

Annual Family Income Program Cost
Over $125,000 $5,470
$100,000-$124,999 $4,678

We will have you sign a formal statement as to your financial status if you use the sliding scale fee rates, and a copy of last year's W-2 form is needed:  Insurance cannot be mixed with sliding fee scale rates.

Other therapeutic activities, included in the cost, will also be conducted. There are no other program charges.
